About our Hawthorne Location

Harmony Bay is an outpatient behavioral health service that provides primary mental health treatment programs for psychiatric conditions such as depression, anxiety, bipolar disorder, and PTSD. We offer a clean, bright environment with a calming and positive tone. In addition, our mental health and wellness treatment center focuses on providing you with a new outlook on life through a blend of traditional and integrative wellness services.

Michelle Peresipa, APN, MSN, PMHNP-BC

Michelle takes time to truly listen to her patients’ stories and blends evidence-based treatment with compassion and respect, empowering her patients to take an active role in their own care. She values collaboration, tailoring treatment plans to each person‘s unique needs and goals, and strives to create a safe, supportive environment where healing can begin.

Stephanie Rahinsky, LCSW

Stephanie has always known she wanted to help others, and social work was her way to make that dream come true. I am looking forward to assisting clients who may be dealing with life changes, loss, trauma, and are ready to make a change to help themselves.

Alexis Segre, LSW

Alexis has worked in crisis intervention and crisis counseling, serving runaway and homeless youth as well as serving children with various emotional disturbances.

Ashley Kwafo, APN, PMHNP-BC

Ashley Kwafo, APN, is a Board Certified Psychiatric Mental Health Nurse Practitioner. Her educational experiences at Wilkes University, coupled with her occupational experiences in various inpatient and outpatient settings, continue to heighten her compassion in mental health care.

Cassaundra Bracken, LCSW

Cassaundra is passionate about supporting people on their journey of self-discovery and empowerment. Cassaundra utilizes a person-centered, strengths-based, and trauma-informed approach.

Tara Reynolds, MS, LPC

Tara Reynolds sees the counseling process as a metamorphosis from a caterpillar into a butterfly. During this process, one is uncovering and unlearning all that is not helpful.
